NY Department of Health has released a request for applications (RFA) for the Creating Healthy Schools and Communities grant award, available here and due January 7, 2015 by 4 PM. The DOH web site location of the RFA and additional information is here.

The DOH seeks applications “to prevent obesity in New York State. The aim… is to fund a coordinated, multi-sector effort to increase demand for and access to healthy food and opportunities for physical activity, to reduce the risk of obesity in high-need communities and school districts.” It anticipates 25 of these awards will be made, with a maximum of $250,000/year for five years. Awards will be distributed statewide, according to regions noted in the RFA. Only high-need communities are eligible to participate, and school districts of the community are required partner(s) in the proposal. The RFA contains additional information and requirements.

Beginning work on your proposal now, meeting with required partners, and completing the required memorandums of understanding (MOUs), will help ensure a successful application for your community.
